Everything you need before your visit.
What to bring, how insurance works, our appointment policies, and your rights as a patient — in one place.
New patients
Please arrive 15 minutes early for your first visit, or complete your forms online from the link we text you.
Bring with you
- Photo ID
- Insurance card (front and back)
- All medications and supplements, in their bottles
- Recent lab results or records
- Names of your other doctors
- Any forms that need to be completed
Patients under 18 must be accompanied by a parent or legal guardian.
Insurance & payment
We participate with many commercial plans and Medicare for preventive visits, chronic care, and labs. Networks change, so please call before your first visit and we'll verify your coverage.
- Copays and deductibles are collected at check-in.
- Self-pay patients receive a written good-faith estimate before scheduled services, as required by the federal No Surprises Act.
- IV hydration and most coaching are self-pay wellness services.
- We accept cash, major credit and debit cards, and HSA/FSA cards.
Appointments
- Book by phone at (305) 558-4192 or through our contact form.
- If you're more than 15 minutes late, we may need to reschedule so other patients aren't delayed.
- Please give at least 24 hours' notice to cancel. Repeated no-shows may lead to a no-show fee for self-pay services; see our Refund Policy.
- After hours, our voicemail lists an on-call number for established patients with urgent questions. For emergencies, call 911.
Telehealth
Established patients can have many follow-up visits by secure video, including weight-management check-ins, lab reviews, and chronic-care follow-ups.
- You must be physically in Florida during the visit.
- We'll text a secure link 10 minutes before your appointment — no app download required.
- Some needs, like physical exams and labs, still require an in-person visit.
Records & your rights
- Request copies of your records in writing; most requests are completed within 14 days.
- Your health information is protected under HIPAA. Our Notice of Privacy Practices is available at the front desk and on request.
- You have the right to an interpreter at no cost. We provide Spanish in-house and arrange others by phone.
- You have the right to respectful care, to participate in decisions about your care, and to refuse treatment.
- Concerns can be raised with our practice manager (see Contact) or with the Florida Department of Health.
